Important HIV Guidelines Update for Prevention and Treatment of Opportunistic Infections in HIV/Infected Adults and Adolescents.
Released by the Centers for Disease Control on March 23, 2009

Today (March 23, 2009), the Centers for Disease Control and Prevention's (CDC) Morbidity and Mortality Weekly Report (MMWR) is posting online the 2009 Guidelines for Prevention and Treatment of Opportunistic Infections in HIV-Infected Adults and Adolescents: Recommendations from CDC, the National Institutes of Health, and the HIV Medicine Association of the Infectious Diseases Society of America (officially published April 10, 2009 as MMWR Volume 58, Reports and Recommendations No. 4). This publication will replace the draft version of the Guidelines posted electronically on June 18, 2008 at www.aidsinfo.nih.gov.  The purpose of this letter is to alert you of the dissemination of the Guidelines and to describe its content, as well as provide information on where to obtain copies.  A parallel version of these guidelines for HIV-infected infants and children will be forthcoming shortly under a separate announcement. 
 
In the United States, the incidence of HIV-associated opportunistic infections (OIs) has declined dramatically, largely due to the advent of simpler, more potent, and less toxic antiretroviral therapy.  However, we should not be falsely reassured that OIs are soon to become a thing of the past. Despite declines in their incidence and major improvements in their prevention and treatment, OIs remain a leading cause of hospitalization and death for persons living with HIV infection. Because most of the existing clinical workforce was trained in OI management during an earlier era when these infections were more prevalent, it is critical that resources such as these guidelines be available for newer clinicians who may have had less experience with these infections but are likely to encounter them during their career. 
 
These Guidelines are intended for use by clinicians and other health-care providers, HIV-infected patients, and policy makers in the United States.  They were developed by a panel of more than 140 experts from the U.S. government and academic institutions. These Guidelines combine and replace two separate MMWR publications: the 2002 Guidelines for preventing opportunistic infections among HIV-infected persons: recommendations of the US Public Health Service and the Infectious Diseases Society of America and the 2004 Treating opportunistic infections among HIV-infected adults and adolescents: recommendations from CDC, the National Institutes of Health, and the HIV Medicine Association/ Infectious Diseases Society of America.

The 2009 Guidelines address OIs that occur in the United States and five OIs that may be acquired during international travel.  Topic areas covered for each OI include: prevention of exposure, prevention of disease by chemoprophylaxis and vaccination, discontinuation of primary prophylaxis after immune reconstitution, treatment of disease, monitoring for adverse effects of treatment and disease, management of treatment failure, prevention of disease recurrence, discontinuation of secondary prophylaxis, and special considerations during pregnancy and with regard to immune reconstitution inflammatory syndromes. For each OI, a small group with subject matter expertise reviewed the literature for new information since the previous guidelines' publication and proposed revised recommendations at a meeting at the National Institutes of Health (NIH) in May 2007. Following these presentations and discussion, the revised guidelines were further reviewed and approved by NIH, CDC, and HIV Medicine Association (HIVMA) of the Infectious Diseases Society of America (IDSA).

We could not accomplish this task without the collaboration of NIH and the HIVMA, as well as the many persons who volunteered to participate in the development of these Guidelines.  We hope you will find them a valuable source of information to maximize prevention strategies and to promptly recognize, accurately diagnose, and properly treat and manage OIs in order to ensure that HIV- infected patients continue receiving the highest quality care possible in the United States.

As President Obama prepares to go before the American people at tonight's prime-time press conference, it is worth noting the growing inconsistencies between his soaring rhetoric on the campaign trail and his actions so far as President.
Click below to see a video highlighting a few of the contradictions between Candidate Obama and President Obama. Candidate Obama promised to take a "scalpel" to wasteful earmarks in the federal budget but President Obama recently signed the pork-laden omnibus bill. Candidate Obama pledged to reinstate "pay-as-you-go" budgeting but President Obama's recent budget proposal will leave a whopping $9.3 trillion deficit over the next decade. 

President Obama's actions clearly do not represent the change that he promised Americans on Election Day. The public is angry about these enormous government spending bills - especially in light of the scandal where taxpayer dollars were used to pay out excessive bonuses to failed American Insurance Group (A.I.G.) executives. Meanwhile, there are growing questions among members of the President's own political Party regarding the size and scope of his proposed budget.
While you may not be able to ask a question at the press conference as President Obama makes his case to the American people tonight, keep in mind the difference between campaign pledges and broken promises.

On Fox & Friends, John Fund advanced the Republican falsehood that Democrats created the right for AIG to pay bonuses by passing the economic recovery bill. In fact, the recovery bill did not create the right for AIG -- or any company -- to pay bonuses. In fact, OMB director Peter Orszag recently said that deficits at the level estimated by CBO in scoring Obama's proposed budget "would ultimately not be sustainable," but also said: "I think that what you're going to see, again, under our assumptions, our policies lead to lower deficits than that." Orszag also recently asserted that "[t]he President's Budget," if enacted, would "put the nation on a sustainable fiscal path."

A Washington Times article reported GOP criticism of Democrats over the AIG bonus issue and quoted a Republican strategist asserting: "This is not something [Democrats] can point to George Bush. ... They own the issue of giving bonuses to the AIG executives." But the article did not note that $53 million in AIG bonuses that the article mentioned were reportedly paid out under the Bush administration, or that a Bush-appointed special inspector general for TARP has stated that the Bush Treasury Department knew about the AIG bonus contracts and did not insist on their abrogation as a condition of AIG's receiving bailout money. NY Times falsely claimed that Obama "campaigned as an antiwar candidate"
In a New York Times article, Sheryl Gay Stolberg wrote that President Obama "never served in the military and campaigned as an antiwar candidate." In fact, Obama did not campaign as "an antiwar candidate"; Obama has repeatedly said that he doesn't "oppose all wars" but is opposed to "a dumb war" or "a rash war." Read More
Politico published Bauer op-ed that advanced 61-detainee falsehood
Politico published an op-ed by Gary Bauer that falsely claimed that "a January Pentagon report found that 61 former detainees [of the prison at Guantánamo Bay] had engaged in terrorist activities." In fact, according to the Pentagon, 18 former Guantánamo detainees have been "confirmed" to have "return[ed] to the fight," while an additional 43 former detainees are "suspected" of having done so. Even the Pentagon's "confirmed" figure has been questioned by analysts. Read More
Wall Street Journal article omitted Bush Treasury Department's role from AIG bonus timeline
A Wall Street Journal article about Tim Geithner and his aides' involvement in decisions about AIG's bonus payments did not note that it was the Bush administration that negotiated a November 2008 stock purchase agreement with AIG through which the Bush Treasury Department injected $40 billion into the company without requiring that the bonus contracts be nullified. Following Politico's lead, media fixate on Obama's "awkward laughter" in 60 Minutes interview
Echoing a March 22 Politico article that was hyped by the Drudge Report, the March 23 editions of Today, Morning Joe, MSNBC Live, and Fox & Friends all featured segments on President Obama's laughter during a 60 Minutes interview. The segments are reminiscent of the media's echoing Drudge by seizing on Hillary Clinton's laugh as a new subject of attention following Clinton's talk show appearances in September 2007. Read More
Did O'Reilly violate his own ambush rules?
According to the O'Reilly Factor website, Bill O'Reilly will address "why ... the far left, in cahoots with NBC News, hurt a rape victim and her family." The post appears to be a reference to an "ambush" interview of the Center for American Progress' Amanda Terkel reportedly conducted by Factor producer Jesse Watters. But O'Reilly has previously suggested that the "ambush journalism" conducted by his producers is limited to targeting "public servants" and stated that prior to those "ambush" interviews, subjects would either be asked on the program or asked "to issue a clear statement explaining their actions." So how did we get to this point? Job losses in the hundreds of thousands every month, almost weekly bank failures, and 401(k)s at half their value from a year and a half ago. The media treat the question as a mystery that can be solved through scrutiny of the Obama administration's actions starting on January 20, 2009 -- an approach that is absurd on its face. The media's erasure of the Bush administration and its policies in their coverage of the economy has been so pervasive that they have given round-the-clock attention to the AIG bonus scandal for days on end with virtually no mention of the fact that it was the Bush administration that last fall approved billions of dollars in aid to AIG without requiring the company to nullify its bonus contracts.
